Understanding the cost of care should be simple and transparent. At Calibrate Physical Therapy, we offer both insurance-based and cash-pay options to make high-quality, one-on-one physical therapy accessible to our patients in Loves Park, Rockford, and surrounding communities.

Insurance We Currently Accept

We are currently in-network with the following insurance providers:

  • Blue Cross Blue Shield (BCBS)
  • Medicare
  • United Healthcare
  • TRICARE West
  • Workers’ Compensation

We are continually working to expand our in-network partnerships so that we can best serve the community.  If you do not see your insurance listed, we encourage you to contact us.  We may still be able to work with your plan or provide out-of-network options.

Cash Pay/Self Pay Options:

We offer transparent, affordable self-pay pricing for patients who prefer not to use insurance or who have out-of-network plans.

  • Initial Evaluation: $100
  • Follow-up Treatment Visits: $80

Cash pay is a great option for patients seeking flexible, one-on-one care without the limitations or visit caps sometimes associated with insurance plans.  We offer multiple forms of payment for your convenience, including cash, credit card, debit card, HSA (Health Savings Account), and FSA (Flexible Spending Account).

Superbill & Out-of-Network Benefits

For patients utilizing cash-pay services or those with out-of-network insurance benefits, we can provide a detailed Superbill upon request.

A Superbill is an itemized receipt that includes the necessary treatment codes required by insurance companies. Patients may submit this document directly to their insurance provider to request potential reimbursement for physical therapy services, depending on their out-of-network benefits.  Reimbursement may be all, partial, or applied to your annual deductible.

Please note that reimbursement is determined by your insurance plan, and payment is not guaranteed. We recommend contacting your insurance provider to confirm your out-of-network physical therapy benefits.

Pricing & Insurance FAQ

Do I need a referral to start physical therapy?
In most cases, no referral is required due to direct access laws in Illinois. However, some insurance plans may require a referral for coverage, so we recommend checking with your insurance provider or contacting us to verify.

How do I know if you are in-network with my insurance?
We are currently in-network with BCBS, TRICARE West, and Medicare. We are actively adding new insurance plans, so please contact us to confirm your specific coverage.

What if you are not in-network with my insurance?
You can still receive care at Calibrate and may use out-of-network benefits.  We provide superbills that you can submit to your insurance for potential reimbursement or application to your annual deductible.

Will insurance cover 100% of my visits?
Coverage varies by plan.  Many patients have deductibles, copays, or coinsurance that affects their out-of-pocket cost.  We recommend verifying your benefits before your first visit.

Why would someone choose cash pay instead of insurance?
Cash pay may be preferred by patients that have high insurance deductibles, limited visit caps, or those who want more flexibility in their treatment without insurance restrictions.

Do you accept workers’ compensation cases?
Yes, we accept workers’ compensation patients. Please contact us prior to your visit so we can verify authorization and required documentation.
